Evaluation Process for Adults

Evaluation Process for Adults

Therapy OfficeAfter a free consultation call, we will schedule an intake session to learn about your concerns as well as take a brief history leading up to what has brought you to treatment. Your therapist will assess your needs and develop an appropriate plan of treatment through the use of an art therapy assessment.

Parent Information

Assessment Process for Children: To do great therapy, it is important to complete a thorough assessment of new clients. We recommend the following assessment process in order to gain as much information about your child as possible in order to be able to provide the best treatment possible:

Recommended Child Therapy Evaluation

  1. Parent Meeting:
After a free consultation call, we will schedule a parent-only intake meeting. This gives us the opportunity to talk openly about your child and your concerns. During this intake we will take a developmental history as well discuss what brought you to treatment.
  2. Family Art Therapy Assessment:
A family assessment helps us understand family dynamics; it is helpful for us to understand how your child fits into the family system by observing their interactions with siblings and caregivers. Artistic skill is not needed to participate in this assessment.
  3. Individual Art Therapy Assessment:
During the individual art therapy assessment, we will ask your child to complete a series of drawings and discuss them. From this assessment, we will learn about their coping skills, their developmental level, and other important information that is not typically revealed through verbal communication alone.
  4. Parent Feedback Session and Treatment Planning:
The feedback session includes a review of our findings from the assessment process. We take a collaborative approach in which we work with parents to discuss a plan of treatment, as well as develop goals to meet the needs of their child.

Custody

The therapists at Moonrise do not participate in custody hearings, custody/access disputes, visitation, or divorce proceedings. The aforementioned circumstances are out of our scope of practice. Therapists participating in these types of matters can be detrimental to the therapeutic relationship and therapeutic process. Please contact a custody evaluator to assist you with these types of concerns.

Fees and Insurance

The Moonrise Art Therapy Collective does not take insurance and is out of network with all providers. We provide monthly superbills which can be submitted to your insurance company for reimbursement. We are not responsible for communicating with your insurance company in regards to reimbursement. We strongly recommend you contact your insurance company prior to beginning treatment to find out if and how much they will pay. Sliding scale fees are available on a case by case basis. We accept all major credit cards through our secure electronic medical record system, Simple Practice.
